Do You Need AZSA Forms?
Members, is it time to reorder leases or other forms? Simply click on the Retail Store tab and the system will take you through the ordering process. Or, to use the updated Member Order Form - including our new address and sales "tax" - simply click here. This will help ensure your order is processed in a timely way.
AZSA Members can order all the forms we offer.
Non-Members, if you'd like to order the Declaration of Sale form (the only one available unless you are an AZSA member) please complete and return the special NON-Member order form. Payment must be received before orders are fulfilled. Orders are typically mailed from the post office once a week; they are not available for pick-up.
Has Your Membership Information Changed?
In order to better serve you, please remember to update any changes to your profile if you move, change phone/fax numbers/email, address, primary contact, etc. It's quick and easy: Click on the "Members Only" tab of the homepage, enter your Username and Password, make the revisions on Your Profile, scroll to the bottom of the page, and click "Submit Registration." Don't miss out on receiving your benefits of belonging -- help us help YOU and let us know what's new!
