Become a Member:
Need AZSA Forms:
- Members, is it time to re-order leases or other forms?
- Membership in AZSA entitles you to ALL FORMS.
- Click the Retail Store tab to order the forms your need, or CLICK HERE to use the updated Member Order Form (including our new address and sales "tax")
- The "Declaration of Sale" form is the only AZSA form available to non-members. Non-members may order it by completing the NON-MEMEBER ORDER FORM
- (Payment must be received before orders are fulfilled. Orders are typically mailed from the post office once a week; they are not available for pick-up.)
Membership Information Changed:
In order to better serve you, please remember to update any changes to your profile if you move, change phone/fax numbers/email, address, primary contact, etc.
- Click on the "Members Only" tab and sign in to access your member profile.
- Make all necessary revisions to Your Profile
- Click "Submit Registration" at the bottom of the page
NOTE: If your member profile is out of date, you may miss out on important information and benefits!
